New Patient Packet

Your First Visit

We're thrilled that you have chosen Carolina Women's Health! To help your first visit run smoothly, please complete the information in our new patient packet prior to your appointment and bring it, and your medical insurance card, with you to your first visit. We request that you arrive about 15 minutes early so that we can complete your records. For location information and driving directions, please visit our Contact page.  

Canceling or Rescheduling Appointments

If you are unable to keep your scheduled appointment, please call us as soon as possible to cancel or reschedule. There is a $25 fee for missed appointments and appointments that are cancelled or rescheduled with less than 24 hours notice.

If you have any medical questions, we encourage you to call us at (864) 382-4000. A nurse will call you back as soon as possible after talking with your physician.  You may also want to review our list of Patient FAQs for general inquiries.

Refilling a Prescription

To request a prescription refill, please call us at (864) 382-4000. A nurse will call you back as soon as possible after consulting with your physician. Please note that there is a charge to refill prescriptions and a charge to call in a new prescription.

Insurance Information

We accept most insurance providers. Please contact our office prior to your appointment to verify acceptance. Current providers include: Blue Cross/ Blue Shield, Companion, Cigna, Aetna, Medcost, United Healthcare and private pay.
